WHAT IT MEANS
Understanding the concern
Heart failure means the heart is not pumping as effectively as it should. It does not mean that the heart has stopped. Breathlessness, tiredness and swelling can occur, but these symptoms also have other causes and need assessment.
When to discuss an evaluation
- Follow-up after a diagnosis or hospital admission
- Changes in activity tolerance, swelling or weight
- Questions about medicines, monitoring and when to seek help
PREPARE FOR YOUR VISIT
Bring the full picture.
Previous reports, your medicine list and a short record of symptoms can make the discussion more useful.

YOUR NEXT STEP
What to expect
Bring discharge summaries, echocardiogram reports and a current medicine list. Describe changes since your last review. Ask for a clear plan for follow-up and what to do if symptoms worsen; treatment depends on the type of heart failure and your overall health.
Questions worth asking
- What type of heart failure have I been diagnosed with?
- What should I monitor at home?
- Which changes require urgent review?
Ask for an explanation in words you understand, and check who to contact if you need help after the appointment.
